How was the app created?

The Change of Condition Application was built by Y Point Analytics in partnership with Minuteman Senior, based on feedback from Minuteman leadership, volunteers and caregivers.

This app is currently being used by around 700 volunteers who have delivered over 200,000 meals to 1,400 seniors in isolated neighborhoods.

Route Planning

Our route planning algorithm optimally distributes consumers between a number of drivers that are available from each meal distribution site. It takes linear and circular routes into consideration and tries to ensure that the same driver delivers meals to the same consumer. This allows drivers and consumers to build strong relationships increasing the likelihood of drivers capturing change in the health conditions of consumers. Our algorithm works across 100’s meal distribution sites and 1000’s consumers. It can take linear or circular routes, or a hybrid mix of routes into consideration prior to re-distributing consumers between drivers. It allows site coordinators to react to staffing shortages by rapidly adjusting the number of drivers available.

Route Optimization

Our route optimization algorithm sequences deliveries for minimum time or minimum distance. This results in significant time and gas savings for drivers and maximizes their efficiency of drivers.

Follow-up Actions

Our mobile app requests drivers to respond to customized health condition-specific questions during each delivery. In the event a driver detects a change in the health condition of a senior, our mobile app allows the driver to submit a “change of condition” notification. These notifications are distributed to care managers and care coordinators in real-time, allowing a rapid response.

Subsequently, our application tracks follow-up action items from the SAMS database and automatically follows up with care managers to ensure proper follow-up action items are tracked.

Our app parses text in the care manager/care provider notes to automatically create follow-up action items for care providers.

There will be an increase in service fees by 3% each year, to account for inflation.

*Note: The number of consumers is based on the total number of consumers who delivered at least one meal for that billing month. If a consumer in our application was not delivered any meals during the billing month, this consumer will not be counted for billing purposes.
